Sexual Dysfunction and Infertility Amongst Spouses in Adamawa State, Nigeria

Abstract: Deterioration in reproductive health and family food sufficiency leading to increase in marital crisis or divorce, arising from sexual dysfunction and infertility amongst spouses in Nigeria is alarming. An investigation was carried out to find the causes, signs and symptoms of sexual dysfunction and infertility amongst spouses in Adamawa State, Nigeria. Respondents were purposively selected with the use of questionnaires for oral interviews, observations and discussions. One hundred volunteer spouses were screened and sent to hospitals for diagnosis of reproductive diseases through culture of their urine, stool, vaginal swabs, semen and prostate content. These specimens were also examined under the electric microscope. Ultrasounds scanning of prostate glands, bladders, kidneys and ovaries or fallopian tubes of the spouses were also carried out to check for abnormalities. The results showed that, 88% of the spouses suffering from these conditions are youths (the active work force of the nation) between the ages of 20 to 40years while 12% are in the age bracket of 50 to 70years old. The findings indicated that, 90% of them are married while 10% are still single. The results showed that, 78% of the spouses had infection for 1 to 10years while 22% had it for more than 11years. The results showed that, 80% of them contracted the infection through sexual intercourse while 20% don’t know how and where they got infected. The hospital culture of urine, vaginal swabs, semen and prostate content of all the spouses showed negative results. The microscopic examinations of these specimens yielded positive to abdominal Schistosomiasis (bilharziasis). The ultrasound scanning of the prostate glands, kidneys and ovaries/ fallopian tubes revealed abnormalities of various degrees. Spouses with early or mild infections (for 1 to 10years) manifested the first 15 symptoms while those with severe and critical infections (for more than 11years) manifested all the 30 symptoms. Therefore, infections of the urinary bladders, genital organs, prostate glands and kidneys by Schistosome species contracted through sexual intercourse may be responsible for sexual dysfunction and infertility amongst spouses in the study area. There should be proper sensitivity for the diagnosis of mild infections caused by this parasite. Immediate authorities should, as a matter of urgency, initiate and support the development of a drug in commercial quantity that could be used to serve as an intervention programme in Nigeria and the world at large.

Abstract: Healthy jokes are one of the best ways to cope with life's pressures. The humor is relieved or prevented by accepting the fleeting self-esteem of a hidden desire, which creates pleasure or happiness, while at the same time alleviating the anguish that often manifests the miracle of a wish. The research method was descriptive-survey. The statistical population of this study included all teenage girls and boys of Zahedan in the academic year of 2019 and the sample was selected by random sampling method &using Cochran formula which was 270 people. The standardized questionnaires used for data collection included Andrews & Parker Stress Coping Strategies Questionnaire (1990) and Humorous Styles Questionnaire of Martin et al. (1988). Spss25 software was used for data analysis. In this study, the demographic status of the subjects was examined by age and gender. According to the descriptive statistics, among the dimensions of stress coping styles, the highest mean was related to problem-oriented style and among the dimensions of humor styles, the highest mean was related to self-destructive style. The results of the data analysis confirmed all the existing hypotheses and showed that there is a relationship between coping styles and humor styles in Teenagers.

Low-Density Lipoprotein Cholesterol Levels Among Individuals Receiving Statin Therapy: Real-World Evidence from India

Abstract: Despite the established clinical effectiveness of statin therapy, a substantial proportion of patients fail to attain the target low-density lipoprotein cholesterol (LDL-C) levels and remain at risk for cardiovascular events. This study aimed to evaluate the proportion of patients achieving the guideline recommended LDL-C levels in real-world settings after receiving statins for primary and secondary prevention of atherosclerotic cardiovascular disease (ASCVD) in India. The study included a cross-sectional retrospective analysis of medical records from 2281 private healthcare facilities between 2017 and 2018. Overall, 15879 patients aged 20-80 years irrespective of their ASCVD status were included. Mean (±SD) age of patients was 55.96±10.41 years; 62.8% were men, and 44.6% (n=7076) had clinical ASCVD. Overall, 96.2% (n=15271) patients were receiving statins, 99.3% in the secondary prevention and 93.6% in the primary prevention cohort. Most patients were receiving moderate-intensity statins for primary (89.7%, n=7391) and secondary ASCVD prevention (73.4%, n=5159). None of the patients in the secondary prevention cohort achieved the recommended LDL-C level of <70 mg/dL. Approximately 25.3% (n=2089) individuals in the primary prevention and 20.2% (n=1418) in the secondary prevention cohort achieved LDL-C <100 mg/dL. Similar proportion (23.2%, n=3361) of patients with LDL-C control (<100 mg/dL) were found among the high-risk coronary heart disease (CHD) or CHD-equivalent group (including those with diabetes). This large real-world study demonstrated levels of LDL-C that were higher than guideline recommended targets, especially among ASCVD patients, despite receiving statin therapy. The results highlight major gaps in the real-world practice of prescribing statin therapy for both primary and secondary prevention of ASCVD. Concordance to guideline recommended therapy, timely dose titration, use of alternative drugs, and patient adherence can bridge this gap and help achieve optimal control of LDL-C. Further intensification of therapy with addition of non-statins is recommended if LDL-C goals are not achieved among high-risk population.

Abstract: Background: To facilitate improvement in participation, therapy needs to be transferred into everyday life of people with chronic stroke. Individualised, home based, self-training exercise programs should be oriented towards the potential of the person, be specific to their ability and impairment levels, avoid compensation strategies as far as possible and operate at the upper limit of the individual’s ability level. Objectives: To investigate the effectiveness of an individualised learning and exercise program based on the Bobath concept for facilitating personal goal achievement in people with chronic stroke. Method: Repeated measures design. Fifty-five people with chronic stroke were recruited from participants in Advanced Bobath training courses. During the five day course, participants learned individualised self-training programs, which they then carried out at home for three months, adapting their program according to their performance level. The primary outcome measure was the Canadian Occupational Performance Measure (COPM) - Performance domain. Secondary measures included the COPM satisfaction domain, World Health Organisation Disability Assessment Score-2 (WHO-DAS2), and Goal Attainment Score (GAS). Measurements were taken three months and immediately prior to the Bobath course and three months after the course. Results: COPM-Perfomance, COPM-Satisfaction and the WHODAS-2 showed a significant difference over the time points, with followup scores significantly greater than both baseline and preintervention (p<.001). Sixty eight percent of participants achieved their personal goals (GAS). Conclusion: The study indicates that an individualised self-training approach based on Bobath principles may enable the transfer of acquired proficiency into the personal goals of people with chronic stroke.
